The structure that works is consistent: a worked example the pupil can refer back to, then fluency questions, then application, then something genuinely harder for those who get there. What people get wrong
No worked example
A worksheet that opens with questions assumes the pupil already remembers. One worked example at the top saves half the hands going up.
Flat difficulty
Twenty questions at the same level teaches nothing after the fifth. Difficulty should climb.
Differentiation as an afterthought
A single sheet with "extension: do question 20 twice" isn't differentiation. Scaffold and stretch need writing in.
